Bring your filled out evaluation questionnaire. (Contact us so we can email that to you). If you have not provided a copy of your insurance card, please present that at your first appointment. We’ll need to take a copy of the front and back of your card to keep on file. If you have a doctor’s referral, we strongly recommend that you submit this in advance of your initial appointment.
