Setting up the appointment
When you inquire about our services online or on the phone, we’ll set up an initial appointment with our therapists within the next day or two. When confirming the appointment on call, you can tell us your preferred date and time for the booking.
Do I need to fill out any forms?
To save time, download, print out, and bring these preliminary forms to your first appointment. These will be used to review your history, ensure consent, and obtain necessary billing details and payment information.
You will need the following forms for your appointment:

- Consent Form
- HIPAA Form
- Company Policy
- Transportation Waiver
- Insurance Form

What should I wear?
Physical therapy movements are highly varied, so it’s best to wear clothing you feel comfortable moving around in. We recommend lounge-wear or workout gear that’s both easy to exercise in and comfortable for you.
If you’re experiencing pain in any specific region, the clothing should allow easy exposure of the area so the physical therapist can evaluate it easily.
Do I need to bring anything?
You need to bring your insurance information and any valid form of photo identification, such as a driver’s license. Bringing filled forms is optional.
How long will the appointment be?
The first appointment session will last approximately 60–90 minutes, and you can expect subsequent sessions to be similarly long. If you haven’t completed the forms, plan to arrive about 20 minutes prior to the scheduled time.
